What happened in Production section:
DONG handover documentation: While running the 13 3/8″ casing, the casing tong was not lowered all the way down, in the start position on the tracks after M/up a connection. So when the casing hands backed the casing tong back in under the PRS cabin the casing tong was too high to get under PRS cabin resulting in the casing tong hitting the motor on windshield wiper for the PRS cabin and braking some of the bolts holding the motor in place. The motor was immediately secured with a cargo strap. So there was no potential for it to fall down.The casing tong could have broken all the bolts holding the motor for the PRS windshield wiper, resulting in the motor would have fallen down on top of the casing tong. No risk for personnel in that area sins they are operating the casing tong from the other side of the rig floor.Casing tong was not lowered all the way down, in the start position on the tracks after M/up a connection. So when the casing hands backed the casing tong back in under the PRS cabin the casing tong was too high to get under PRS cabin resulting in the casing tong hitting the motor on windshield wiper for the PRS cabin.

What Was Learned

Tell all casing hands coming out on Maersk Giant about this incident, so this can be prevented from happening in future casing operations. Comments: The PRS cabin is going to be moved on the yard stay, so this will no longer be an issue after the yard stay. Ref DONG Synergi report no 41043.
