Negative Operational LL with DNO

Oselvar (A-1 H, A-2 H, A-3 HT2) – P&A

What happened:

Had only 1 Dolphin sub sea engineer. He had to work overtime, and OIM had to make deviation. The sub sea worked more than 16 hrs in one stretch. His presence was on the critical path of the operation.

What was planned:

Land the lower marine riser package, disconnect the BOP from x-mas tree, function test BOP, etc. Lots of BOP work.

What was learned:

It was not sufficient with only 1 sub sea engineer onboard during this phase of the operation

What actions are required:

Consider having 2 subsea engineers (24 hr coverage) in P&A operations where BOP is jumped between well and lifted off HXT and wellhead frequently.
