What happened in P&A:
The 9 5/8″ casing hanger had a 10″ pup joint made up to it This was not identified on the hanger schematic. The 10″ could not be handled with the rig or franks elevators and could not be broken out with the franks tong. The assembly needed to be layed down as a 19 m long piece which was awkward and time consuming.

What Was Planned

The plan was to use 9 5/8″ inserts on the iron roughneck.

What Was Learned

Old tallies are not always 100% correct.

What Actions Are Required

For wells A-2 and A-1 it is recommended to get 10″ elevator inserts and casing jaws.
