What happened in P&A:
1 hr extra rig time was unnecessarily spent to make up x-over to the RT before it could be made up to the drill pipe. Lots of manual work for personnel in red zone, hands inside the iron roughneck, personnel in riding belt on top of the iron roughneck, etc. etc.

What Was Planned

Make up mechanical running tool for 13 3/8″ EZSV bridge plug to the running drill pipe

What Was Learned

The RT had one extra box-box NC50 XO made up to it, making in longer and more top heavy than necessary. The length made it difficult to fit it into the iron roughneck, standing on deck. It was not possible to set RT in rotary and use slips on it.

What Actions Are Required

In the planning phase onshore, find out what type of DP will be used to run a bridge plug, and then order out a RT with the correct thread that will match the DP thread. In this case we new that we would spot a balanced cmt plug on top of the 13 3/8″ EZSV plug, and then we always use 3 1/2″ DP (not 5 7/8″ DP). Still the RT was sent out with NC50 threads, something we dont use at all on the Borgland Dolphin.

What Was Done

The rig had to make up another rig XO from NC50 to NC38 which is the tread for the 3 1/2″ DP cmt stinger. This was hard to do without using manual rig tongs, we barely managed to do it with the iron roughneck.
