What happened in Top hole:
Equipment was not loaded onto rig when rig was moored at Gomez well location. Offloading supply boat was attempted when all 8 anchor lines had been disconnected, and still much swell from bad weather 24 hr earlier.

What Was Planned

Get spud equipment onboard rig before spud. This mean 36″ hole drilling equipment & mud, plus 30″ conductor & cement. This enable crew to check out equipment before spud, and to make casing tally and space out for the BHA.

What Was Learned

It was hazardous and time consuming to work supply boat due to much relative movement between supply boat and rig. Supply boat captain would not move boat close to rig, and rig crane had to lower boom far out to reach boat. It took 4 hrs to load Barite bulk, and get the 36″ drilling equipment and mud chemicals only. Were not able to get the conductor. The 36″ WH housing was a heavy lift, 16 MT.

What Actions Are Required

Best solution is to load and unload equipment from rig before AH starts. Second best solution is to mark what is primary and back-up equipment on the manifest, so that only primary lifts are taken up. The primary equipment and heaviest lifts should be placed on the center of the boat deck, where movements are smaller. The conductor tally should be made up onshore, and sent to DSV. We can not RIH with 36″ BHA before the conductor has been measured and tally made. This information is needed for marking 36″ BHA on surface, so conductors must come to rig at same time as 36″ BHA.
