What happened in Top hole:
The PSV boat was sent without the 9 7/8″ Pilot BHA due a mistake in WELS. The loadout day of the BHA had expired and it was therefore not picked up by the base/boat/onshore/offshore personnel to have it out.

What Was Planned

To have the 9 7/8″ Pilot BHA on boat from Kristiansund. In the meetings, both morning and afternoon, it was emphasized that the 9 7/8″ pilot hole was going to be shipped on next boat.

What Was Learned

Important to update loadout dates and always double, or tripple, check the manifest from the boat that leaves as soon as possible.

What Actions Are Required

To update the loadout dates continously during operation and let the base know which critical equipment that is going on the boat. It is also important to send the boat before midnight so the drilling engineer offshore can go through the manifest of the boat before he goes to bed. In that way mistakes can be found quick and actions can be taken immediately.

What Was Done

Luckily the mistake of a missing 9 7/8″ BHA was observed in the manifest just after the boat departing. Boat was then sent back yo base to pick it up and no major consequences happened.
