Grosbeak West (35/11-21 S) – Reservoir section
What happened:
During P/U weak link bails, the electric remote control was switched off on the shute. Due to the shut off of electric remote control, the shute “moped” started to move downwards with weak link bails in shute. The movement of the shute stopped due to bolted weak link clamp on WL bails hit the edge on the shute. Fastened tugger on weak link bail and checked for hydraulic leaks which were not found.
What was planned:
Securing weak bails during P/U and R/D
What was learned:
Do not switch off electric remote control panel during using of shute during P/U heavy equipment. This is a safety reminder involved personnel shall know.
What actions are required:
Use tugger when lifting up weak link bails as an extra barrier to avoid unattended movement of shute.
