What happened in Intermediate section:
The remote operated lo-torq valve was installed with an elbow between side inlet and lo-torq, while the Archer cmt. Head was laying on the catwalk machine. The cmt head was then picked up halfway to vertical. Then it was decided to install the cmt hose. Cmt head was attempted laid back down, but lo-torq valve got in the way and a tugger was used to turn the cmt head prior to laying all down. 2 ” cmt hose was installed and cmt head picked up to vertical. While making up cmt head to joint in rotary table; a tugger wire was used in an attempt to stop the side entry sub from rotating with the cmt stand, but got wrapped around 2 turns. Tugger wire was un-tackled and the cmt head racked back. Prior to use of cmt stand, a small part from the cement stand was observed dropping inside the red zone on drill floor/ set back.

What Was Planned

Installation of remote operated lo-torq valve on cmt head and M/U cmt stand.

What Was Learned

The adaptor and the position indicator should be made in one piece (Archer to follow up).
