What happened in P&A:
Unable to run Weatherford 9 5/8″ casing cuting assembly past wellhead. Found out that spiral stabilizer above casing cutter was 9.5″ instead of 8.5″. Wrong size stab was made up to pre-made assembly onshore. Weatherford engineer and Transocean AD failed to spot the error on the rig. Weatherford did arrive late on rig and had to measure tool in a hurry.

What Was Planned

Cut 9 5/8″ casing

What Was Learned

The rig did not have amy bit gauges to measure OD of spiral stab. Need to get PDC and rock bit gauges from bit supplier for all bit sizes on next well. Weatherfor and Transocean need to get gauge that can measure spiral stab.Normally the DD has this gauge, but he had left rig with gauge.

What Actions Are Required

Get Wtfd to check the OD of the quipment before sending out. Include the OD in the BHA schematics. Include gauge available in Se. Rec. Or DOP.
