Negative Equipment LL with PGNiG

Shrek (6507/5-9 S, 6507/5-9 A) – Surface section

What happened:

Unable to make up Dril-Quip 13 3/8” no cross coupling (VAM 21) to WH assembly (VAM 21).
Had to break off 1 m saver sub joint on WH assembly and replaced no-cross coupling joint with a regular joint. Had to change out box on connection jt below no cross jt as well.

What was planned:

M/U Dril-Quip 13 3/8″ no cross coupling to 18 3/4″ WH assy (both VAM 21).

What was learned:

After an investigation by independent parties it was clear that the driller had been overly eager when running and installing the wellhead, as too much weight was put down, effectively ruining the threads on the connection.

What actions are required:

Driller to take it slow and steady and stay within recommended paramters at all times. It should be clearly written in RAP/DOP to be mindful of this.
