What happened:
Lifting incident: when laying down the FA-1 tool came in contact with the Expro tong and damaged the tong due to failed sling (overloaded). Had to replace the casing tong.
What was planned:
L/O the FA-1 tool to due to incompatibility with the casing
What was learned:
Incorrect lifting plan for laying out tool.
What actions are required:
Review lifting plans and procedures to ensure lifting equipment is not overloaded. Load in sling can change (increase) dramatically when load path changed from vertical to horizontal.
