What happened in Rig move:
When work wire and pre-laid mooring line was retrieved after vessel had received rig PCP, centerpiece of kenterlink was missing.

What Was Planned

Hook up pre-laid mooring line to AHV`s work wire using standard type kenterlink with locking pin and led

What Was Learned

Deck crew forgot to insert led to secure the locking pin from falling out.

What Actions Are Required

Only new lock kenterlinks to be used in future operations. These kenterlinks do not have a locking-pin or need of using led. The kenterlink is secured with a locking-screw in the centerpiece.
If required amount of new lock kenterlinks can`t be delivered, pictures should be taken of standard led kenters showing that locking pin is secured using led.

What Was Done

New equippment supplier has been selected for the upcomming operation at Hemispheres.
Only New fast lock kenter links will be used in the mooring lines at Hemispheres.
Led kenter links may still be used for buoy off and lay down operations as this is not critical to the mooring line when rig is moored.
