What happened:

Inner bushing with cut-out for wireline was not mobilized for this wireline conveyed logging job. A set of bushings was ordered the day before logging commenced, but could not be lifted onto rig due to weather. The rig modified their own inner bushings, and grinded a groove into the bushings.

What was planned:

Pipe conveyed logging with slotted inner bushing.

What was learned:

Certified slotted bushings proved inferior to rig modified items. This because the slot was wider than required and spanned the ends of the two halves, which allowed the cable to be slightly flattened. Would be better to design a slot “just wide enough” for the cable in use.

What actions are required:

Schlumberger should flag the requirement at the job planning stage.
