What happened:
Difficulties to install the double drilling machine at scaffolding at diverter level for cutting the lifting eyes for lifting the 18 MT cut conductor after fine cut performed. Sent out new inserts to the drilling machine used for cutting the drain holes to easier fit in the cutting area for the lifting eyes
What was planned:
Use a double drilling machine to cold cut lifting eyes to connect Claxton lifting pin/bridle for lifting the cut conductor. Slings connected to bails to support the lifting pin/bridle.
What was learned:
Review procedure for drilling holes for lifting pin for lifting away 18 MT cut stub after fine cut.
What actions are required:
Implement agreed procedure for next operation. Evaluate to use the same drilling machine as for the drain holes.
