What happened:

We didn’t ask of information from the DSV during the operation

What was planned:

The KPI’ s in the scorecards are sometimes difficult to measure and are dependent on help from the DSV, i.e.:
– QHSE reports ( including Leasson learned) issued to QHSE Advisor (Yes/No/Partial)
– Workplace HSE Participation (incl. HSE mtgs, SJA & Safe Cards)
– Work Site Audits (Inspection with Operator Rep)

What was learned:

We don’t have the information now at the end of the well

What actions are required:

Agree on reporting routines from DSV, i.e. Bi-weekly report from DSV

What Was Done

Will be implemented for the next project
