An evaluation of some changes was not documented.
A MOC process should cover any RELEVANT changes to:
— surface and well control equipment;
— impact on WBEs;
— well type (e.g. conversion from producer to injector);
— procedures;
— rig or contractor well control equipment;
— key personnel;
— any change in design basis or operating condition for the well

Changes related to documents and key personnel

What was learned:

Evaluations of changes to organisation, documents and equipment that does not trigger a MoC process should be documented.

What actions are required:

Focus on Moc i bi-weekly /weekly meetings
Prepare a log where all MoC evaluations are documented
