What happened:

No lessons learnt meetings were held with the offshore team (DSV and DE) during operations.

What was planned:

We did not plan any lessons learnt meetings with the offshore team (DSV and DE).

What was learned:

A lot of new lessons learnt were captured for Oselvar, might have been better to have a short bi-weekly lessons learnt meeting to more easily close out the lessons learnt.

What actions are required:

The DSV on days could participate in the the LL meeting on his hitch so that LL’s were closed out with the relevant team while still fresh in memory. Evaluate to have bi-weekly LL meetings with offshore team.
