What happened:

Monkey fist not attached to wellhead running tool prior to rigging up. Had to wait for ROV to make and connect monkey fist to equipment online.

What was planned:

ROV Monkey fist

What was learned:

Should be done offline, when running tool still is on deck. Both Dril-Quip and Oceaneering have a bulletpoint in the preparation checklist to confirm monkey fist present. Also make sure the rope is long enough so that the ROV can access and grip it when the RT is landed, inside the SMO, with mud on top. If too short ROV is unable to find it if the current stirs up the mud inside the SMO

What actions are required:

Make sure every service company actually reads the dops and preparation checklist prior to operation.
