What happened:
Used correct keys but Expro crew not fully familiar with how to install and remove – broke one key when the shoe track was pulled.
What was planned:
Use anti rotation keys to lock 20” connectors.
What was learned:
Clarify in planning stage who (which service company) is responsible for installing the anti-rotation keys. Ensure the responsible company is trained by Dril-Quip to run and remove anti-rotation keys with the correct tools.
What actions are required:
Review with appropriate coordinator before any future job. Ensure full DrilQuip procedure communicated to them and that its passed on to crews
