Negative Operational LL with Wellesley

What happened:

After unlatching the BOP we forgot to remove the AX ring from the wellhead prior to pulling the BOP. After the BOP was pulled the BOP lifting crane developed a fault which prevented it from being racked. This in turn prevented the main ROV from being launched. Because the AX ring had not been removed, the smaller “eyeball” ROV could not be launched to monitor the wellhead cutting operation.

Unlatch BOP & pull AX ring

What was learned:

One should remove the AX ring ASAP after unlatching, (and to place in a subsea basket, not over a guidepost. This removes the need for over the side moonpool work to retrieve the gasket from the guidepost later.

What actions are required:

Should include reminder to remove AX ring in DOP prior to pulling BOP.
